    HAPI FHIR: XHTML narrative parser unbounded recursion causes StackOverflow denial of service
    Summary
    HAPI FHIR is a complete implementation of the HL7 FHIR standard for healthcare interoperability in Java. Prior to 6.9.11, XhtmlParser.java imposes no maximum element nesting depth, so a deeply nested text.div narrative triggers unbounded recursion between parseElementInner() and parseElement(), raising a StackOverflowError. An attacker who can submit FHIR resources containing such narratives can thus crash a parsing or validation worker thread, affecting validator services and any application that parses attacker-supplied FHIR JSON or XML. This issue is fixed in version 6.9.11.

    HAPI FHIR: JSON utility parser unbounded recursion causes StackOverflow denial of service
    Summary
    HAPI FHIR is a complete implementation of the HL7 FHIR standard for healthcare interoperability in Java. Prior to 6.9.11, the JSON utility parser in org.hl7.fhir.utilities.json.parser.JsonParser enforces no maximum nesting depth for arrays or objects. As a result, a small but deeply nested, syntactically valid FHIR JSON document can trigger unbounded readArray() or readObject() recursion, raising a StackOverflowError before structural validation runs. An attacker who can submit JSON resources for validation can thus crash the request thread, and services that do not isolate StackOverflowError safely may experience worker loss or process instability — a denial-of-service condition. This issue is fixed in version 6.9.11.

    Title
    HAPI FHIR: Stored XSS in scan report via unescaped IG and profile titles
    Summary
    HAPI FHIR is a complete implementation of the HL7 FHIR standard for healthcare interoperability in Java. Prior to 6.9.11, the hidden scan command concatenates attacker-controlled Implementation Guide titles, profile titles, and source references into scan.html without escaping in Scanner.java. As a result, a user who scans an attacker-supplied IG/profile and then opens or publishes the generated local/CI HTML report can trigger stored cross-site scripting, executing attacker-controlled JavaScript in the report's browser context. This issue is fixed in version 6.9.11.

    HAPI FHIR: ReDoS via FHIRPath matches()/replaceMatches() in FHIR Validator HTTP Endpoint
    Summary
    HAPI FHIR is a complete implementation of the HL7 FHIR standard for healthcare interoperability in Java. Prior to 6.9.9 and 6.9.4.2, all implementations of FHIRPathEngine accept arbitrary FHIRPath expressions and evaluate them without input validation, and the FHIRPath functions matches(), matchesFull(), and replaceMatches() pass user-controlled regular expressions to Java's Pattern.compile() and String.replaceAll() through an incomplete timeout utility. An attacker can send a resource containing an evil regex pattern that causes catastrophic backtracking, exhausting CPU resources and causing denial of service in the FHIR Validator HTTP endpoint and affected org.hl7.fhir.* modules. This issue is fixed in versions 6.9.9 and 6.9.4.2.

    HAPI FHIR: ReDoS via FHIRPath matches()/replaceMatches() in FHIR Validator HTTP Endpoint
    Summary
    HAPI FHIR is a complete implementation of the HL7 FHIR standard for healthcare interoperability in Java. Prior to 6.9.7, the FHIRPathEngine implementation passes user-controlled regular expressions from matches(), matchesFull(), and replaceMatches() to Java regex operations without effective timeouts, allowing catastrophic backtracking and denial of service. This issue is fixed in version 6.9.7.

    HAPI FHIR: Unauthenticated SSRF via /loadIG Chains with startsWith() Credential Leak for Authentication Token Theft
    Summary
    HAPI FHIR is a complete implementation of the HL7 FHIR standard for healthcare interoperability in Java. Prior to version 6.9.4, the FHIR Validator HTTP service exposes an unauthenticated "/loadIG" endpoint that makes outbound HTTP requests to attacker-controlled URLs. Combined with a startsWith() URL prefix matching flaw in the credential provider (ManagedWebAccessUtils.getServer()), an attacker can steal authentication tokens (Bearer, Basic, API keys) configured for legitimate FHIR servers by registering a domain that prefix-matches a configured server URL. This issue has been patched in version 6.9.4.

    HAPI FHIR is a complete implementation of the HL7 FHIR standard for healthcare interoperability in Java.     Summary
    HAPI FHIR is a complete implementation of the HL7 FHIR standard for healthcare interoperability in Java. Prior to version 6.9.0, when setting headers in HTTP requests, the internal HTTP client sends headers first to the host in the initial URL but also, if asked to follow redirects and a 30X HTTP response code is returned, to the host mentioned in URL in the Location: response header value. Sending the same set of headers to subsequent hosts is a problem as this header often contains privacy sensitive information or data that could allow others to impersonate the client's request. This issue has been patched in release 6.9.0. No known workarounds are available.

    XXE vulnerability in XSLT parsing in `org.hl7.fhir.core`
    Summary
    HAPI FHIR is a complete implementation of the HL7 FHIR standard for healthcare interoperability in Java. XSLT parsing performed by various components are vulnerable to XML external entity injections. A processed XML file with a malicious DTD tag ( <!DOCTYPE foo [<!ENTITY example SYSTEM "/etc/passwd"> ]> could produce XML containing data from the host system. This impacts use cases where org.hl7.fhir.core is being used to within a host where external clients can submit XML. This is related to GHSA-6cr6-ph3p-f5rf, in which its fix (#1571 & #1717) was incomplete. This issue has been addressed in release version 6.4.0 and all users are advised to upgrade. There are no known workarounds for this vulnerability.

    HAPI FHIR is a complete implementation of the HL7 FHIR standard for healthcare interoperability in Java. Prior to 6.9.11, XhtmlParser.java imposes no maximum element nesting depth, so a deeply nested text.div narrative triggers unbounded recursion between parseElementInner() and parseElement(), raising a StackOverflowError. An attacker who can submit FHIR resources containing such narratives can thus crash a parsing or validation worker thread, affecting validator services and any application that parses attacker-supplied FHIR JSON or XML. This issue is fixed in version 6.9.11.

As a result, a small but deeply nested, syntactically valid FHIR JSON document can trigger unbounded readArray() or readObject() recursion, raising a StackOverflowError before structural validation runs. An attacker who can submit JSON resources for validation can thus crash the request thread, and services that do not isolate StackOverflowError safely may experience worker loss or process instability — a denial-of-service condition. This issue is fixed in version 6.9.11.

    Title
    HAPI FHIR: Stored XSS in scan report via unescaped IG and profile titles
    Summary
    HAPI FHIR is a complete implementation of the HL7 FHIR standard for healthcare interoperability in Java. Prior to 6.9.11, the hidden scan command concatenates attacker-controlled Implementation Guide titles, profile titles, and source references into scan.html without escaping in Scanner.java. As a result, a user who scans an attacker-supplied IG/profile and then opens or publishes the generated local/CI HTML report can trigger stored cross-site scripting, executing attacker-controlled JavaScript in the report's browser context. This issue is fixed in version 6.9.11.

    HAPI FHIR is a complete implementation of the HL7 FHIR standard for healthcare interoperability in Java. Prior to 6.9.9 and 6.9.4.2, all implementations of FHIRPathEngine accept arbitrary FHIRPath expressions and evaluate them without input validation, and the FHIRPath functions matches(), matchesFull(), and replaceMatches() pass user-controlled regular expressions to Java's Pattern.compile() and String.replaceAll() through an incomplete timeout utility. An attacker can send a resource containing an evil regex pattern that causes catastrophic backtracking, exhausting CPU resources and causing denial of service in the FHIR Validator HTTP endpoint and affected org.hl7.fhir.* modules. This issue is fixed in versions 6.9.9 and 6.9.4.2.

    HAPI FHIR: DSTU2 FHIRPathEngine.matches() missing RegexTimeout protection allows ReDoS
    Summary
    HAPI FHIR is a complete implementation of the HL7 FHIR standard for healthcare interoperability in Java. Prior to 6.9.10, the fix for CVE-2026-45367 incompletely patched the DSTU2 module, leaving FHIRPathEngine.matches() in org.hl7.fhir.dstu2/utils/FHIRPathEngine.java to call raw String.matches(sw) without RegexTimeout protection while replaceMatches() was updated, allowing an unauthenticated attacker to trigger catastrophic regex backtracking and exhaust server CPU. This issue is fixed in version 6.9.10.

    HAPI FHIR: Unauthenticated SSRF via /loadIG Chains with startsWith() Credential Leak for Authentication Token Theft
    Summary
    HAPI FHIR is a complete implementation of the HL7 FHIR standard for healthcare interoperability in Java. Prior to version 6.9.4, the FHIR Validator HTTP service exposes an unauthenticated "/loadIG" endpoint that makes outbound HTTP requests to attacker-controlled URLs. Combined with a startsWith() URL prefix matching flaw in the credential provider (ManagedWebAccessUtils.getServer()), an attacker can steal authentication tokens (Bearer, Basic, API keys) configured for legitimate FHIR servers by registering a domain that prefix-matches a configured server URL. This issue has been patched in version 6.9.4.

    Title
    HAPI FHIR HTTP authentication leak in redirects
    Summary
    HAPI FHIR is a complete implementation of the HL7 FHIR standard for healthcare interoperability in Java. Prior to version 6.9.0, when setting headers in HTTP requests, the internal HTTP client sends headers first to the host in the initial URL but also, if asked to follow redirects and a 30X HTTP response code is returned, to the host mentioned in URL in the Location: response header value. Sending the same set of headers to subsequent hosts is a problem as this header often contains privacy sensitive information or data that could allow others to impersonate the client's request. This issue has been patched in release 6.9.0. No known workarounds are available.

    XXE vulnerability in XSLT parsing in `org.hl7.fhir.core`
    Summary
    HAPI FHIR is a complete implementation of the HL7 FHIR standard for healthcare interoperability in Java. XSLT parsing performed by various components are vulnerable to XML external entity injections. A processed XML file with a malicious DTD tag ( <!DOCTYPE foo [<!ENTITY example SYSTEM "/etc/passwd"> ]> could produce XML containing data from the host system. This impacts use cases where org.hl7.fhir.core is being used to within a host where external clients can submit XML. This is related to GHSA-6cr6-ph3p-f5rf, in which its fix (#1571 & #1717) was incomplete. This issue has been addressed in release version 6.4.0 and all users are advised to upgrade. There are no known workarounds for this vulnerability.
