Prior to version 1.0.4, the PIN-type challenge throttle uses `emailHash` as the only key. The throttle rows live in the central `challenge_throttle` table, which is shared across all tenants. Every tenant\u0027s `/api/tenants/{id}/appointments/verify-challenge` endpoint increments the same row when a PIN response fails, and every tenant\u0027s `/api/tenants/{id}/appointments/challenge` endpoint reads the same row when deciding whether to issue a new challenge. When the same `emailHash` exists in multiple tenants on the same OpenReception instance (the same patient holding tunnels in two different clinics that share the platform), an attacker who knows the patient\u0027s email can lock out that patient on tenant B by issuing failed challenge responses against tenant A. The attacker needs no relationship to tenant B; the lockout propagates through the shared throttle row. The lockout escalates with repeated failures. The first lockout triggers at 4 failed attempts and lasts approximately 60 seconds. Subsequent failures escalate the lockout duration to 5 minutes, 30 minutes, and 60 minutes per the throttle service\u0027s escalation logic. Repeated bursts produce sustained denial of service against the targeted email. Version 1.0.4 patches the issue."

In versions 4.38.0 through 4.39.19, when a user authenticates via Basic Auth (i.e via the `Authorization` header with the `Basic` scheme) on the authz verification endpoint, Authelia takes the username directly from the `Authorization` header and passes it as is to the regulation system for ban checking and attempt recording. LDAP treats usernames case insensitively : `john`, `John`, and `JOHN` all bind as the same user. But the regulation SQL queries treat the lookup of these values in certain scenarios as case sensitive. This allows each variation of a usernames case to have its own ban bucket. Upgrade to 4.39.20 to receive a patch. As a workaround, explicitly disable the basic auth mechanism."

Prior to version 0.8.0, the password reset confirmation endpoint `/client/reset-password-confirm/:hash` is handled by a non-API controller and is not covered by FOSSBilling\u0027s rate limiter, which only applies to `/api/*` routes. This allows an attacker to probe the endpoint for valid reset tokens without any per-IP request limiting, attempt counting, or lockout mechanism. The endpoint acts as an oracle, returning a distinguishable response for valid versus invalid tokens (HTTP 200 vs HTTP 302 redirect). An attacker can submit unlimited token guesses to the password reset confirmation endpoint with no throttling applied. However, practical exploitability is significantly mitigated by the current token generation, which uses `hash(\u0027sha256\u0027, random_bytes(32))`, providing 256 bits of entropy. Tokens also expire after 15 minutes and are deleted after successful use. The same architectural gap applies to other controller-served auth routes, including `/staff/email/:hash` (admin password reset confirmation) and `/client/confirm-email/:hash` (email confirmation). Version 0.8.0 fixes the issue. Some workarounds are available. Configure a reverse proxy (e.g., Nginx, Apache, Cloudflare) to apply per-IP rate limiting to the `/client/reset-password-confirm/*` and `/staff/email/*` paths and/or use a WAF rule to limit request rates to these endpoints."

Mitigation
- Common protection mechanisms include:
- Disconnecting the user after a small number of failed attempts
- Implementing a timeout
- Locking out a targeted account
- Requiring a computational task on the user's part.

Strategy: Libraries or Frameworks
- Use a vetted library or framework that does not allow this weakness to occur or provides constructs that make this weakness easier to avoid [REF-1482].
- Consider using libraries with authentication capabilities such as OpenSSL or the ESAPI Authenticator. [REF-45]
CAPEC-16: Dictionary-based Password Attack
An attacker tries each of the words in a dictionary as passwords to gain access to the system via some user's account. If the password chosen by the user was a word within the dictionary, this attack will be successful (in the absence of other mitigations). This is a specific instance of the password brute forcing attack pattern.
Dictionary Attacks differ from similar attacks such as Password Spraying (CAPEC-565) and Credential Stuffing (CAPEC-600), since they leverage unknown username/password combinations and don't care about inducing account lockouts.
CAPEC-49: Password Brute Forcing
An adversary tries every possible value for a password until they succeed. A brute force attack, if feasible computationally, will always be successful because it will essentially go through all possible passwords given the alphabet used (lower case letters, upper case letters, numbers, symbols, etc.) and the maximum length of the password.
CAPEC-560: Use of Known Domain Credentials
An adversary guesses or obtains (i.e. steals or purchases) legitimate credentials (e.g. userID/password) to achieve authentication and to perform authorized actions under the guise of an authenticated user or service.
CAPEC-565: Password Spraying
In a Password Spraying attack, an adversary tries a small list (e.g. 3-5) of common or expected passwords, often matching the target's complexity policy, against a known list of user accounts to gain valid credentials. The adversary tries a particular password for each user account, before moving onto the next password in the list. This approach assists the adversary in remaining undetected by avoiding rapid or frequent account lockouts. The adversary may then reattempt the process with additional passwords, once enough time has passed to prevent inducing a lockout.
CAPEC-600: Credential Stuffing
An adversary tries known username/password combinations against different systems, applications, or services to gain additional authenticated access. Credential Stuffing attacks rely upon the fact that many users leverage the same username/password combination for multiple systems, applications, and services.
CAPEC-652: Use of Known Kerberos Credentials
An adversary obtains (i.e. steals or purchases) legitimate Kerberos credentials (e.g. Kerberos service account userID/password or Kerberos Tickets) with the goal of achieving authenticated access to additional systems, applications, or services within the domain.
CAPEC-653: Use of Known Operating System Credentials
An adversary guesses or obtains (i.e. steals or purchases) legitimate operating system credentials (e.g. userID/password) to achieve authentication and to perform authorized actions on the system, under the guise of an authenticated user or service. This applies to any Operating System.
