The product reuses or references memory after it has been freed. At some point afterward, the memory may be allocated again and saved in another pointer, while the original pointer references a location somewhere within the new allocation. Any operations using the original pointer are no longer valid because the memory "belongs" to the code that operates on the new pointer.

Attackers can trigger the vulnerability by causing qsThreadInfo to fail after qfThreadInfo successfully allocates RDebugPid structures, resulting in double-free memory corruption when the error path attempts to clean up the list.

- vfs_iocb_iter_read
- ext4_file_read_iter
- ext4_dio_read_iter
- iomap_dio_rw : bio was submitted and return -EIOCBQUEUED - dio_aio_complete_work - dio_complete - dio->iocb->ki_complete (erofs_fileio_ki_complete()) - kfree(rq) : it frees iocb, iocb.ki_filp can be UAF in file_accessed().
- file_accessed : access NULL file point
Introduce a reference count in struct erofs_fileio_rq, and initialize it as two, both erofs_fileio_ki_complete() and erofs_fileio_rq_submit() will decrease reference count, the last one decreasing the reference count to zero will free rq.

Smatch report warning as follows:
drivers/staging/vme_user/vme_tsi148.c:1757 tsi148_dma_list_add() warn: '&entry->list' not removed from list
In tsi148_dma_list_add(), the error path "goto err_dma" will not remove entry->list from list->entries, but entry will be freed, then list traversal may cause UAF.
Fix by removeing it from list->entries before free().
